WHAT IS PHYSICAL THERAPY?

The American Physical Therapy Association defines Physical Therapy as the care and services provided by a Physical Therapist to patients who have physical functional limitations resulting from injury, disease, or other causes.

WOULD I BENEFIT FROM PHYSICAL THERAPY?

The answer is yes if…
  • You have fallen or have had a near fall episode.
  • Joint or muscle pain limits daily activities.
  • Poor endurance affects your ability to maintain activities for long durations.
  • You suffer from back pain or sciatica.
  • Normal daily functions (such as, getting up from a chair or bed, walking, bathing, dressing, homemaking, or leaving the home, etc…) have become difficult.
  • You are weak from acute or chronic illness.
  • You are weak following a surgery or hospitalization.
  • Your walking has become unsteady.
  • You have had a stroke.
  • You have a progressive neurologic disorder (Parkinsons Disease, MS, Huntington's Chorea, etc…)
HOW ARE THE SERVICES PAID?

We provide individualized skilled rehab services in the home that are reimbursed by Medicare Part B and most secondary insurance policies. Please contact our office if you have any questions.

DO I HAVE TO MEET MEDICARE HOMEBOUND CRITERIA?

Active for Life Rehabilitation provides Physical Therapy in the home to patients who have difficulty performing normal daily tasks due to physical impairments. Patients do not have to meet the Medicare Part A criteria of being homebound. Active for Life Rehabilitation fills in the gap, providing therapy to patients in their home who find it difficult to travel to appointments, are intimidated by the fast paced environment of out-patient Sports Physical Therapy clinics, or would benefit from a more individualized and focused treatment environment.

HOW DO I GET STARTED?
Physical Therapy, like medication, must be prescribed by an doctor, podiatrist, or Nurse Practitioner. If you already have a prescription, simply call our office to schedule an appointment. If you feel that you could benefit from Physical Therapy, talk to your doctor about your concerns and request that he/she write a prescription if appropriate.

I'VE NEVER BEEN ACTIVE BEFORE, CAN PHYSICAL THERAPY REALLY HELP ME?

Studies have shown that senior adults have more to gain by becoming physically active than younger adults. Although results may take slightly longer, outcomes are often more pronounced in the older adult.

WHAT CAN I EXPECT?

The first step is an initial evaluation. Here your therapist will learn about your present condition and the daily tasks that have been affected. A physical assessment will be performed looking at your posture, flexibility, strength, balance, endurance, and sensation. A Plan of Care will be created and sent to your physician. A treatment schedule will be established. Treatments sessions are typically provided 2-3 times per week for approximately 45 minutes to 1 hour in duration. During each session, the physical limitations identified during the initial evaluation will be treated with the appropriate therapeutic interventions. As your condition improves, treatment frequency is often tapered and a personalized Home Exercise Program Training becomes part of the treatment focus. Treatment ends when you have achieved all goals or a maximum potential is reached.  

MY DOCTOR SUGGESTED I GO TO A PARTICULAR CLINIC, CAN I UTILIZE ACTIVE FOR LIFE REHABILITATION INSTEAD?

Yes. Patients have the right to choose their Physical Therapy provider. We treat a wide variety of conditions, but would be happy to discuss your case with you to determine if we can appropriately address your condition.
